Tanto el esguince como la fractura se acompañan de gran dolor e inflamación, pero la impotencia funcional (incapacidad para mover la zona afectada) y la deformidad son propias de las lesiones óseas. Hasta que se descarte una fractura, los esguinces graves se deben tratar como lesiones óseas; sólo la radiografía puede confirmar el diagnóstico.
En la fractura, el hueso absorbe la energía del traumatismo, perdiendo su integridad estructural. En el esguince el traumatismo es absorbido por una articulación, distendiéndose o rompiéndose las fibras de un ligamento o la cápsula articular. Como un movimiento muy leve produce dolor intenso, no se debe manipular la extremidad afectada ni intentar “enderezarla” ni corregir la deformidad. Esto, además, puede aumentar la lesión de partes blandas, producida por los extremos óseos fracturados al moverse; este fenómeno cobra especial importancia en el caso de los vasos y los nervios. La incorrecta manipulación de un miembro fracturado puede hacer que los picos y biseles de la fractura desgarren arterias, venas o nervios. Sólo se debe inmovilizar el miembro en la posición en que se encuentra, preferiblemente con férulas. Éstas se pueden improvisar con tablas o cartón y afianzarlas al miembro con tiras de tela.
Si la cabeza o el tronco de la víctima se encuentran doblados o torsionados en posición antinatural se debe sospechar inmediatamente de una fractura o luxación de la columna vertebral. De ningún modo se debe intentar corregir la deformidad o mover el tronco. Otros síntomas de lesión vertebral son el dolor agudo en la espalda o el cuello y la parálisis en las extremidades inferiores. Todo accidentado sospechoso de presentar una lesión vertebral debe ser manejado en estricta inmovilidad, transportado “en tabla”, preferiblemente por varios socorristas, y mejor aún sobre una superficie dura y plana (una puerta, por ejemplo).
